Fertility and Anabolic Steroids: What Canadian Men Should Know

How anabolic steroids lower sperm counts, how long fertility takes to return, what helps recovery, and why men who want children should plan ahead.

Anabolic steroids are, in effect, a powerful male contraceptive. Researchers once tested testosterone as birth control precisely because it switches sperm production off so reliably. For men who want children now or later, that makes fertility one of the most important topics to understand before any use, not after.

Key points

  • Outside androgens shut down FSH and LH, which the testes need to produce sperm.
  • Sperm counts often fall to very low levels or zero within a few months of use.
  • Most men recover, but it can take six to twelve months or longer, and recovery is not guaranteed.
  • A semen analysis is the only way to know where you stand.

Why sperm production stops

Sperm are made in the testes under the direction of two pituitary hormones: FSH, which supports the cells that nurture developing sperm, and LH, which drives testosterone production inside the testes. Testosterone levels inside the testes are many times higher than in the blood, and sperm production depends on that local concentration. When outside androgens are taken, the brain detects high hormone levels and stops sending FSH and LH. Testicular testosterone falls even though blood levels are high, and sperm production slows or stops. Shrinking testicles are the visible sign.

What studies show

In contraceptive trials, weekly testosterone injections reduced sperm counts to very low levels in most men within a few months. After stopping, the large majority recovered, with median recovery times of several months. Studies of men using anabolic steroids outside medicine, often at higher amounts and for longer, find slower and less certain recovery. Fertility clinics regularly see men with no sperm in their semen whose only explanation is past steroid use.

Factors that make recovery slower

  • Longer total time using, and repeated cycles without full recovery in between.
  • Higher amounts and multiple compounds.
  • Long-acting esters such as decanoate and undecylenate.
  • Older age and lower fertility before starting.
  • Other factors that harm sperm: smoking, heavy drinking, heat, obesity and some medications.

What can help

For men hoping to conceive after steroid use, the first step is stopping. Doctors who specialise in male fertility sometimes use medications such as clomiphene, HCG or FSH to stimulate recovery; these are prescription treatments with monitoring, and responses vary. HCG used during a cycle to keep the testes active may preserve some function, but it does not guarantee fertility. Product information for HCG and clomiphene lists label uses.

Do not assume you are infertile, or fertile.

Some men conceive while still recovering; others struggle for a long time. Contraception is still needed if pregnancy is not wanted, and a semen analysis is needed if it is.

Planning ahead

Men who think they may want children should consider banking sperm before any use. Sperm freezing is available at fertility clinics across Canada, and a baseline semen analysis shows your starting point. Talking to a family doctor before starting, and being honest about steroid use if fertility becomes a concern, saves time later. Our guide to testosterone suppression and recovery explains the hormone side.

Tests to discuss

Test Purpose
Semen analysis Sperm count, movement and shape
FSH and LH Whether the pituitary has restarted
Testosterone Whether the testes are responding
Estradiol and prolactin Hormones that can stall recovery

Testicle size and feel

Shrinkage usually reverses as production returns. A new lump, persistent pain or a change in shape is different and should be checked by a doctor promptly, regardless of steroid use.

Frequently asked questions

Can I get someone pregnant while using steroids?

It is less likely but possible. Steroids are not reliable contraception.

How long until my fertility returns?

Often six to twelve months, sometimes longer. A semen analysis tracks progress.

Does HCG protect fertility?

It can keep the testes more active, but it does not guarantee normal sperm production.
