The Main Oral Steroids Compared

The main oral anabolic steroids compared side by side: Dianabol, Anadrol, Anavar, Winstrol, Turinabol, Superdrol and Halotestin, with liver and lipid risks.

Oral anabolic steroids are popular because they avoid needles, act quickly and leave quickly. They also share a set of risks that injectables mostly avoid. This guide puts the main tablets side by side: where each came from, what makes it different and what it costs the body, without dosing plans.

Key points

  • Nearly all common oral steroids are 17-alpha-alkylated, which lets them survive the liver and strains it at the same time.
  • They tend to lower HDL cholesterol more sharply than injectables.
  • Short half-lives mean effects stop quickly after the last tablet, but suppression of natural testosterone still needs time to recover.
  • No oral is “safe”; some are simply harsher than others.

The tablets at a glance

Compound Derived from Medical history Known for Main concerns
Dianabol (methandienone) Testosterone Withdrawn decades ago Fast weight, water Estrogen, BP, liver
Anadrol (oxymetholone) DHT Still used for some anemias Strength, weight Liver, BP, blood count
Anavar (oxandrolone) DHT Burns, weight regain Lean strength HDL, liver
Winstrol (stanozolol) DHT Hereditary angioedema Dry, hard look HDL, liver, joints
Turinabol (chlorodehydromethyltestosterone) Testosterone East German sport programme Slow, steady gains Liver, HDL
Superdrol (methasterone) DHT Never approved; sold as a supplement Strong dry gains Severe liver risk
Halotestin (fluoxymesterone) Testosterone Low testosterone, some cancers Strength, aggression Liver, mood, HDL

Why liver strain is shared

Tablets are absorbed through the gut and pass through the liver before reaching the rest of the body. The liver would break most steroids down on that pass, so chemists added an alkyl group at the 17-alpha position. It protects the molecule, but it also interferes with bile flow and liver-cell function. Raised liver enzymes are common; cholestasis, peliosis hepatis and tumours are rarer but serious. The more potent the oral, generally the greater the strain, which is why Superdrol and Anadrol are treated with particular caution. Read more in liver health on steroids.

Why cholesterol suffers more with tablets

Because orals reach the liver in concentrated form, they disrupt how it makes and clears lipoproteins. HDL can fall by half or more within weeks with some compounds, and LDL often rises. That pattern reverses when the drug stops, but repeated exposure adds to long-term cardiovascular risk. Our guide to steroids and cholesterol explains what to look for on a lipid panel.

A few compound notes

Superdrol

Methasterone was never approved as a medicine. It was sold in the 2000s as a “designer” supplement until regulators acted after reports of severe liver injury. It remains one of the riskiest orals for the liver.

Turinabol

Turinabol is best known from the East German state sports programme, where athletes were often given it without consent. Long-term health problems among those athletes are well documented.

Halotestin

Fluoxymesterone is extremely androgenic with little muscle-building effect. It is associated with aggression and significant liver strain.

Who should avoid oral steroids altogether

Anyone with liver disease, high cholesterol, heart disease, high blood pressure, or who drinks heavily or takes medicines processed by the liver has strong reasons to avoid them. People on blood thinners should know that anabolic steroids can increase their effect sharply.

One oral at a time, and briefly.

Combining two orals multiplies liver and cholesterol strain. Medical use of these drugs was usually short or tightly monitored for that reason.

Monitoring

Before and during use: ALT, AST, GGT and bilirubin; a lipid panel; blood pressure at home; a complete blood count; and testosterone to gauge suppression. Our oral vs injectable guide explains how the risks compare with injections.

Frequently asked questions

Which oral steroid is easiest on the liver?

Among 17-alpha-alkylated orals, oxandrolone is usually regarded as less harsh, but none is free of liver strain. Proviron and oral Primobolan use a different structure and are gentler.

Do oral steroids shut down natural testosterone?

Yes, even with short use. Recovery after stopping takes time and is best confirmed with bloodwork.

Why are orals taken more than once a day?

Most have half-lives measured in hours, so levels fall within a day.

Educational information only. Nothing here is medical advice or a recommendation to use any product. Anabolic steroids are Schedule IV under the Controlled Drugs and Substances Act. Talk to a doctor and get bloodwork before and after using any compound.
