Of all the changes anabolic steroids cause, cholesterol is one of the most consistent and one of the least felt. HDL, the protective kind, falls. LDL, the harmful kind, often rises. Nobody notices, yet over years this pattern is one of the main ways steroid use raises the risk of heart attack and stroke. Here is how to read the numbers.
- Almost all anabolic steroids lower HDL; oral 17-alpha-alkylated compounds lower it the most.
- LDL often rises at the same time, which worsens the ratio that matters for heart risk.
- Levels usually recover within weeks to a few months after stopping, but repeated exposure adds up.
- A fasting or non-fasting lipid panel before, during and after use shows what is really happening.
A quick guide to cholesterol
Cholesterol travels in the blood inside lipoprotein particles. LDL particles carry cholesterol to tissues, and when there are too many they lodge in artery walls and build plaque. HDL particles help carry cholesterol away from tissues back to the liver. That is why high LDL and low HDL are the combination doctors worry about.
What steroids do
Androgens increase the activity of an enzyme in the liver called hepatic lipase, which breaks down HDL. They also change how the liver makes and clears other lipoproteins. The effect is strongest with oral compounds, which reach the liver in concentrated form. Studies in healthy men found HDL falling by a third to more than half within weeks on compounds such as stanozolol and oxandrolone. Injectable testosterone at medical levels lowers HDL much less.
| Type of compound | Typical effect on HDL | Effect on LDL |
|---|---|---|
| Testosterone at medical levels | Small decrease | Little change |
| Higher-dose injectables | Moderate decrease | Often increases |
| Oral 17-alpha-alkylated steroids | Large decrease | Often increases |
| SARMs | Dose-dependent decrease | Variable |
Reading a lipid panel
A standard panel reports total cholesterol, LDL, HDL and triglycerides, often with non-HDL cholesterol. Many labs and doctors in Canada also look at apolipoprotein B (ApoB), which counts the number of harmful particles and is a strong predictor of risk. Lipoprotein(a), a mostly genetic risk factor, is worth measuring once in life. Rather than chasing a single number, compare your results before and during use; the change tells you how strongly your body is reacting.
Each period with low HDL and high LDL adds to plaque build-up. Years of repeated cycles can leave lasting damage even if each set of results returns to normal afterwards.
How fast levels recover
After stopping, HDL and LDL usually move back toward baseline over several weeks to a few months, faster after orals than after long-acting injectables. A follow-up panel two to three months after the last dose shows whether recovery is complete.
What helps
- Choose wisely: avoiding oral steroids is the single biggest lever.
- Cardio: regular aerobic exercise modestly raises HDL and improves overall risk.
- Diet: more fibre (oats, beans, vegetables), less saturated and trans fat, and regular oily fish.
- Weight and waist: reducing visceral fat improves the whole lipid profile.
- No smoking: smoking lowers HDL and multiplies artery damage.
Supplements marketed for cholesterol vary widely in evidence. Omega-3 fats mainly lower triglycerides. If LDL or ApoB are high, a doctor may discuss medication.

Frequently asked questions
Which steroids are worst for cholesterol?
Oral 17-alpha-alkylated compounds such as stanozolol, oxandrolone, methandienone and oxymetholone usually lower HDL the most.
How long after stopping does cholesterol recover?
Usually several weeks to a few months. A repeat lipid panel confirms it.
Do I need to fast for a lipid panel?
Many Canadian labs accept non-fasting samples. Follow the instructions on your requisition, and test the same way each time for fair comparisons.
