Drostanolone, sold as Masteron, is a modified form of dihydrotestosterone (DHT). It was once a hospital drug for advanced breast cancer and is now mostly known among competitive bodybuilders for a dry, hard look. It neither builds large amounts of muscle nor converts to estrogen, and both facts shape how people use it and what it costs them.
- Drostanolone is DHT with a methyl group added at the 2-position, which makes it more stable and more anabolic than DHT itself.
- Only the propionate form was approved for people, historically 100 mg three times a week for breast cancer; the enanthate form never had human labelling.
- It does not convert to estrogen and has mild anti-estrogenic activity.
- As a DHT derivative it can speed up hair loss in those prone to it and lowers HDL cholesterol.
From oncology wards to stage prep
Drostanolone propionate was introduced in the late 1950s to treat breast cancer in postmenopausal women, working partly by opposing estrogen. Better hormonal cancer drugs replaced it, and it disappeared from pharmacies. Its later reputation comes from bodybuilding, where it is used close to competitions because it seems to improve muscle hardness at very low body fat.
Propionate or enanthate
Masteron comes on two esters. The propionate is short-acting and clears within days; the enanthate is long-acting and lingers for weeks. The hormone is identical. The choice is about injection frequency and how quickly effects stop after the last dose.
Why the “hard look” depends on body fat
Drostanolone’s visual effect is subtle. It does not cause water retention, because it cannot become estrogen, and it may reduce the estrogenic effects of other compounds slightly. In lean people that can make muscle appear denser. At ordinary body-fat levels, most people notice little. It is a weak mass builder compared with testosterone or nandrolone.
Side effects
Hair
Because it is a DHT derivative, drostanolone acts strongly on hair follicles. Anyone with a family history of male-pattern baldness may see shedding accelerate. Finasteride does not help here, because drostanolone is already in its DHT-like form. Our article on hair loss explains why.
Cholesterol and blood pressure
HDL falls and LDL may rise, as with most anabolic steroids. Blood pressure can increase. These changes are silent and only show up in bloodwork and cuff readings.
Prostate and skin
DHT-type compounds stimulate prostate tissue and oil glands, so acne and prostate symptoms are possible, particularly in older men.
Suppression
Drostanolone suppresses natural testosterone and sperm production like other anabolic steroids.
Because it is strongly androgenic, drostanolone carries a high risk of virilisation for women: voice change, body hair and clitoral enlargement can become permanent.
Where it sits among DHT derivatives
Several anabolic steroids are built on DHT: drostanolone, stanozolol, oxandrolone, oxymetholone and mesterolone among them. What separates drostanolone is that it is injectable and not 17-alpha-alkylated, so it does not carry the heavy liver load of the oral DHT derivatives. That does not make it gentle on cholesterol or hair, but it does mean liver enzymes are usually less affected than with Winstrol or Anadrol tablets.
Testing and detection
Drostanolone is on the World Anti-Doping Agency prohibited list. The propionate clears within days and the enanthate over weeks, but metabolites can remain detectable longer than the drug itself. Anyone subject to testing should assume a long window.
Realistic expectations
Because its effect is mainly cosmetic in very lean people, drostanolone is one of the compounds where expectations and reality differ most. At typical body-fat levels, diet, training and sleep will change how someone looks far more than this compound will.
Bloodwork to discuss with a doctor
Lipid panel, complete blood count with hematocrit, liver and kidney markers, testosterone and estradiol, and PSA for men over 40. Blood pressure at home weekly.

Frequently asked questions
Is Masteron an anti-estrogen?
It has mild anti-estrogenic activity, but it is not a replacement for medical estrogen control and should not be relied on for that.
Will Masteron make me bigger?
Not much. It is a weak mass builder; its effects are mainly visual in lean people.
Why does Masteron cause hair loss?
It is a modified DHT, the hormone that drives male-pattern baldness in genetically prone people.
