Acne is one of the most visible side effects of anabolic steroids. For some people it means a few spots; for others it means painful cysts across the back and chest that can scar. The cause is straightforward, the basic fixes are cheap, and stronger treatment is available when needed.

Key points

  • Androgens enlarge oil glands and increase sebum production, which feeds acne bacteria and inflammation.
  • The back, chest and shoulders are hit hardest because they have dense oil glands.
  • Benzoyl peroxide washes, gentle exfoliation and showering after training help many people.
  • Severe or scarring acne deserves a doctor; isotretinoin works well but needs medical monitoring.

Why androgens cause acne

Oil glands in the skin are packed with androgen receptors. Testosterone and especially DHT make the glands larger and more productive. Extra sebum, combined with skin cells that stick together in the pores, creates blocked follicles where the bacterium Cutibacterium acnes thrives. The immune response to that bacterium produces red, swollen spots and, in severe cases, deep cysts.

Which compounds are more likely to cause it

Any androgen can trigger acne. DHT-derived compounds such as drostanolone, stanozolol and mesterolone, and very androgenic ones such as trenbolone, are frequently mentioned. Higher overall androgen levels mean higher risk. Acne often appears within weeks of starting and settles over a few months after stopping.

Everyday measures that help

  • Benzoyl peroxide wash (2.5 to 5 percent) on the back and chest, left on for a minute before rinsing. It bleaches towels and shirts.
  • Salicylic acid products to keep pores clear.
  • Shower soon after training and change out of sweaty clothes.
  • Clean bedding and towels regularly.
  • Avoid heavy, oily creams on acne-prone areas; look for non-comedogenic labels.
  • Do not pick or squeeze cysts, which increases scarring.
Give treatments time.

Topical products take six to eight weeks to show their full effect. Changing them every few days makes it hard to know what works.

When to see a doctor

Painful lumps under the skin, acne that leaves marks or scars, or acne that does not improve after two months of good skincare are reasons to see a family doctor or dermatologist. Options include topical retinoids, prescription antibiotics for a limited time, and isotretinoin for severe or scarring cases.

Isotretinoin needs supervision

Isotretinoin (sold in Canada under several brand names, and known widely as Accutane) shrinks oil glands and is the most effective acne treatment available. It also carries serious risks: it causes severe birth defects, so pregnancy must be avoided; it raises triglycerides and liver enzymes; it can cause very dry skin and lips, nosebleeds and joint aches; and mood changes have been reported. Doctors monitor blood tests during treatment. Combined with anabolic steroids, which also stress lipids and the liver, the case for medical supervision is even stronger. Our Accutane product page lists the label information.

Do not combine isotretinoin with alcohol or other liver stressors

Both isotretinoin and oral steroids affect the liver and blood fats. Blood tests during treatment are how doctors keep this safe.

What does not help much

Harsh scrubs, alcohol-based toners and drying the skin out aggressively often make acne worse by irritating it. Toothpaste, lemon juice and other home remedies can burn the skin. Sunburn may temporarily dry spots but damages skin and darkens marks. Steady, gentle routines beat dramatic fixes.

Back acne and training gear

Tight synthetic shirts, backpacks and lifting belts trap sweat and rub against the skin, which can worsen acne on the back and shoulders. Breathable clothing, washing gym gear after each use and wiping down benches before lying on them all reduce the friction and bacteria that keep spots coming.

Scarring

Treating acne early is the best way to avoid scars. Existing scars can be improved with treatments such as microneedling, chemical peels and lasers after the acne has settled.

Frequently asked questions

Will steroid acne go away after stopping?

Usually it improves over a few months as hormone levels normalise. Scars may remain.

Why is acne worse on my back than my face?

The back and chest have many large oil glands that respond strongly to androgens.

Does diet affect acne?

High-glycaemic foods and some dairy may worsen acne in some people, but hormones are the main driver with steroid acne.
