How SARMs Work: The Science Explained

How SARMs work: androgen receptors, tissue selectivity, why they were developed, what trials found for ostarine, ligandrol and RAD-140, and their known risks.

Selective androgen receptor modulators were invented to solve a medical problem: how to give the muscle- and bone-building benefits of testosterone to people who are wasting away, without the prostate, skin and hair effects. The science behind that idea is genuinely clever. Here is how it works, what the trials found and where the evidence stops.

Key points

  • SARMs bind the same androgen receptor as testosterone but change its shape differently, which alters which genes it switches on in each tissue.
  • They were developed for muscle wasting, osteoporosis, cancer-related weight loss and similar conditions.
  • Several reached early or mid-stage human trials; none has been approved.
  • Selectivity is partial, not total: suppression and cholesterol changes still occur.

The androgen receptor in brief

Testosterone works by binding androgen receptors inside cells. Once bound, the receptor moves into the nucleus and, together with helper proteins called co-regulators, switches specific genes on or off. Different tissues have different mixes of co-regulators, which is one reason the same hormone does different things in muscle, bone, skin and the prostate.

How selectivity works

A SARM fits the androgen receptor’s binding pocket but holds it in a slightly different shape from testosterone. That shape attracts some co-regulators and not others. In muscle and bone, the combination still produces a strong anabolic signal; in the prostate and skin, the signal is weaker. SARMs are also not converted by the 5-alpha-reductase enzyme into a stronger form, which testosterone is in the prostate and scalp, and they cannot become estrogen. Together these explain why animal studies showed muscle and bone gains with less prostate growth.

The main compounds

Compound Original research aim Furthest stage reached
Ostarine (enobosarm, MK-2866) Cancer-related muscle loss, later breast cancer Phase 2 and phase 3 programmes, not approved
Ligandrol (LGD-4033) Muscle wasting, hip fracture recovery Phase 1 and 2
Testolone (RAD-140) Muscle wasting, breast cancer Early-phase cancer trials
Andarine (S-4) Muscle and bone Preclinical; abandoned
YK-11 Laboratory compound No human trials

What the trials found

In older adults and patients with cancer-related weight loss, ostarine increased lean body mass by roughly one to one and a half kilograms over a few months and improved stair-climbing tests in some studies. Ligandrol in healthy young men increased lean mass by about a kilogram over three weeks at very low amounts, while lowering testosterone, SHBG and HDL cholesterol. The amounts used in these trials are far smaller than those common outside research.

Where selectivity falls short

Selectivity is about the androgen receptor in peripheral tissues. The brain and pituitary still read SARMs as androgens, so natural testosterone production is suppressed. The liver still changes how it handles cholesterol, so HDL falls. Several case reports describe liver injury. Andarine was associated with visual changes, including a yellow tint and trouble adjusting to darkness, in user reports. Our comparison of SARMs and steroids sets these risks beside those of anabolic steroids.

No approval means no reference dose.

Without approved labels, any amount quoted for SARMs comes from forums, not medicine. Bloodwork is the only way to see how your body is responding.

Why none has been approved

Drug development requires showing a meaningful clinical benefit, not just a change in lean mass on a scan. Several SARM programmes struggled to show that patients felt or functioned better enough to justify approval, and some sponsors redirected their compounds toward cancer indications. That history is worth remembering when SARMs are marketed as proven.

Detection and sport

All SARMs are prohibited by the World Anti-Doping Agency, and athletes have been sanctioned for ostarine and ligandrol, sometimes from contaminated supplements.

Frequently asked questions

Are SARMs steroids?

No. They are non-steroidal molecules, though they act on the same receptor.

Do SARMs convert to estrogen?

No, so estrogen-related water retention is not expected. Suppression of natural testosterone can still lower estradiol.

Why is RAD-140 studied in breast cancer?

Androgen receptor activation can slow some hormone-receptor-positive breast cancers, which led to early trials.
