Anavar (Oxandrolone): What It Is and What to Expect

Anavar (oxandrolone) explained: its medical history, why it is called mild, what the label said, liver and cholesterol risks, women and bloodwork.

Oxandrolone, sold for decades as Anavar and later Oxandrin, has a reputation as the gentle steroid. Part of that is earned: it is less androgenic than most and was prescribed to children, women and burn patients. Part of it is a myth, because “mild” never meant harmless. Here is the balanced picture.

Key points

  • Oxandrolone is an oral derivative of DHT, modified so it survives the liver and does not convert to estrogen.
  • Its original label for adults was 2.5 to 20 mg a day in divided doses, usually for two to four weeks at a time.
  • It lowers HDL cholesterol strongly, even at modest amounts, and strains the liver like other oral steroids.
  • It is the compound most often used by women, but virilising effects can still be permanent.

A real medicine with a real history

Oxandrolone was approved in the 1960s to help people regain weight after surgery, severe infection or trauma, and later to reduce muscle loss in burn patients and in HIV-related wasting. Clinical trials in severe burns found faster healing and better preservation of lean mass. It has since been withdrawn from the US market and is no longer a routine pharmacy drug in North America, but the trial data remain one of the better evidence bases for any anabolic steroid.

Why it is called mild

Three features give oxandrolone its reputation. It cannot be converted to estrogen, so it does not cause water retention or gynecomastia through that route. It binds the androgen receptor in a way that favours muscle over androgenic effects such as body hair and voice changes. And it was used safely enough in children and women that the doses in medicine were small.

Where “mild” stops being true

Cholesterol

Oxandrolone is one of the strongest suppressors of HDL cholesterol among anabolic steroids, because oral steroids act directly on the liver’s lipid handling. Big drops in HDL and rises in LDL can appear within weeks, which adds up over repeated use. Our guide to steroids and cholesterol explains how to read a lipid panel.

The liver

Like other oral anabolic steroids, oxandrolone has a 17-alpha-alkyl group that protects it from being broken down in the liver. That same feature makes it a liver stressor. Raised liver enzymes are common; rarer problems include cholestasis and, with long use, liver tumours.

Suppression

It still switches off the body’s own testosterone production. Lower amounts suppress less, but suppression is not zero.

Women and oxandrolone

Oxandrolone is the compound in our range most commonly used by women, and it was prescribed to women in medicine. Even so, a deeper voice, enlarged clitoris, body or facial hair and acne can appear, and voice changes in particular may not reverse. Our article on women and anabolic steroids covers the warning signs.

Stop at the first sign of voice change.

Hoarseness or a lower pitch can become permanent. It is the clearest signal to stop and see a doctor.

Bloodwork to discuss with a doctor

A lipid panel before and a few weeks into use, liver enzymes (ALT, AST, GGT and bilirubin), blood pressure, and for men a testosterone level to gauge suppression. Women may also want a baseline hormone panel.

What to expect realistically

Oxandrolone does not produce dramatic weight gain. In trials, gains were measured in a few kilograms of lean mass over weeks, mostly in people who were losing muscle to begin with. In healthy lifters the typical effect is strength and a harder look at a stable weight, which is why it is associated with cutting.

Taking tablets: absorption and splitting

Oxandrolone is well absorbed by mouth and has a half-life of roughly nine to ten hours in younger adults, longer in older people. That is why the original label split the daily amount into two to four doses. Food does not stop it working, but taking it at consistent times keeps blood levels steadier. If tablets need splitting, a proper pill cutter gives far more even halves than a knife.

Interactions worth knowing

Oxandrolone strengthens the effect of warfarin and similar blood thinners, sometimes dramatically, and the label warned that doses of those drugs often need to be cut. It can also change blood sugar control in people with diabetes. Anyone taking prescription medication should tell their doctor before using any anabolic steroid, including this one.

Frequently asked questions

Is Anavar safe for the liver?

It is less liver-toxic than some orals but still a 17-alpha-alkylated steroid. Liver enzymes should be checked before and during use.

Does Anavar cause gynecomastia?

It does not convert to estrogen, so the usual estrogen route is absent. Gynecomastia from other causes is still possible.

Why are Anavar tablets more expensive than other orals?

Oxandrolone is harder and costlier to synthesise than most oral steroids, which is reflected in the price everywhere.
