Anadrol and Dianabol are the two oral steroids most associated with fast size and strength. They are often treated as interchangeable, but they are different molecules with different histories and different routes to the same well-known problems: water retention, blood pressure and liver strain. This comparison lays them side by side.
- Dianabol (methandienone) is a testosterone derivative; Anadrol (oxymetholone) is a DHT derivative.
- Dianabol converts to a potent estrogen through aromatase; Anadrol does not aromatise but still causes estrogen-like effects.
- Both are 17-alpha-methylated and both stress the liver; Anadrol is generally considered the harder of the two.
- Both raise blood pressure and lower HDL, and both suppress natural testosterone quickly.
The basics
| Anadrol (oxymetholone) | Dianabol (methandienone) | |
|---|---|---|
| Derived from | DHT | Testosterone |
| Medical status | Still approved for some anemias | Withdrawn decades ago |
| Label reference | 1 to 5 mg per kg a day for anemia | About 5 mg a day historically |
| Aromatises to estrogen | No | Yes, to a potent form |
| Approximate half-life | Around 8 to 9 hours | A few hours |
| Liver strain | High | Moderate to high |
Two routes to water retention
Dianabol is converted by aromatase into 17-alpha-methylestradiol, an estrogen that the body clears slowly and that strongly promotes sodium and water retention. That is the classic “Dbol bloat”. Anadrol cannot be aromatised, yet users report similar water retention and even gynecomastia. Researchers think oxymetholone or its metabolites act on the estrogen receptor directly. The practical difference is that an aromatase inhibitor may reduce Dianabol’s estrogen effects but often does little for Anadrol’s.
Strength and weight
Both produce rapid early gains in scale weight, much of it water and glycogen, along with strength increases users notice within a couple of weeks. Anadrol is generally regarded as the stronger per tablet. Neither builds lasting muscle faster than a sound diet and training plan can keep; when they are stopped, the water component leaves, and some of the weight goes with it.
Liver
Both carry a 17-alpha-methyl group that protects them from breakdown in the liver and strains liver cells in return. Raised liver enzymes are common with either. The Anadrol label specifically warns about peliosis hepatis, liver tumours and cholestatic jaundice with prolonged use, which is why it is usually regarded as the harder of the two on the liver. Combining two orals multiplies the strain.
Using two 17-alpha-alkylated orals together compounds liver and cholesterol effects. Liver warning signs include yellow eyes or skin, dark urine, pale stools and pain under the right ribs.
Blood pressure and blood count
Water retention from either drug raises blood pressure. Anadrol adds a second factor: it was developed to increase red blood cell production, so hematocrit can climb too. Both effects increase cardiovascular strain and are silent until measured. Our guide to tracking blood pressure at home explains a reliable routine.
Cholesterol
Oral steroids act directly on the liver’s handling of lipoproteins, so both drugs lower HDL and raise LDL quickly. Short exposure limits the cumulative damage; repeated exposure adds up.
Which is “better”?
There is no safer choice between them, only different trade-offs. Dianabol’s effects are estrogen-driven and partly manageable through that route; Anadrol’s are stronger and harder on the liver and blood count. Both are short-use compounds that need liver, lipid and blood-pressure monitoring. Full profiles: Dianabol and Anadrol.

Frequently asked questions
Which causes more bloating, Anadrol or Dianabol?
Both commonly cause water retention. Dianabol’s is estrogen-driven; Anadrol’s comes from a different mechanism and can be just as marked.
Is Anadrol harder on the liver than Dianabol?
Generally yes, and its label carries specific warnings about serious liver complications with prolonged use.
Can an aromatase inhibitor stop Anadrol water retention?
Often not, because oxymetholone does not work through aromatase.
